If you're unable to complete the digital form, you can instead print and fill out this CLIENT INTAKE FORM.
Or we can provide a paper intake form for you to complete when you arrive.
Stay Hydrated After Your Massage
Massage stimulates fluid movement and increases circulation, similar to light exercise, which can be dehydrating. Proper hydration is essential following a massage to help your lymph system eliminate waste. Drinking enough water post-session helps to prevent soreness and fatigue.
To support your recovery and well-being, we provide every client with a refreshing bottle of water after each session. Remember, please stay hydrated for the best results!

By regularly giving your body the opportunity to operate in a relaxed and efficient state, you train its systems to perform optimally, even in stressful situations. Routine massages play a key role in supporting and sustaining your overall health.

Arrival Time: Please arrive five minutes early. Late arrivals may shorten your session and will still incur the full scheduled fee, as the time has been reserved for you.

Cancellations: Cancel appointments at least 12 hours in advance to avoid a $50 fee. You can cancel online up to 12 hours before your session.

No-Shows: Missing your appointment without notice will be considered a "no-show" and the full session fee may apply.

Contraindications: Massage isn’t suitable for certain conditions like fever, contagious illness, or blood clots. We may decline treatment for safety.

Accepted Forms of Payment: We accept Cash, Zelle, Venmo, Debit/Credit and FSA/HSA cards. While we don’t process insurance claims, we can provide receipts for reimbursement.

Privacy Policy and Confidentiality: Your personal information is strictly confidential and will never be shared with third parties. Conversations with your therapist are private.

Client Boundaries and Feedback: Your comfort and boundaries are respected. Provide feedback about techniques or preferences to ensure the best care.

Comfort and Concerns: Dress as you feel comfortable. You will always be properly draped, and areas like breasts or genitals will not be massaged. Inappropriate behavior will end the session immediately.

Sanitation / Hygiene: Equipment and linens are sanitized after each session. Therapists maintain strict hygiene, and gloves may be used if necessary. Clients should arrive clean and disclose any skin concerns.

Breathing: Deep breathing enhances massage benefits by promoting tension release and muscle oxygenation. It also helps communicate areas of discomfort.

Water Intake: Drink plenty of water post-massage to rehydrate and reduce soreness.

Right to Decline Service: We reserve the right to decline service at any time, at our discretion.
